Future Scenarios for US Physician Practice Under MACRA

American physicians face an increasingly turbulent practice environment. Over the last decade they have experienced the introduction of electronic health records, increased regulatory oversight, new reporting requirements, expanded cost containment initiatives and widespread health system consolidation. The recent MACRA legislation has introduced a new level of regulatory complexity along with financial penalties for practices that perform at below the national average.

This Forum will examine future scenarios for US medical practice. It will review the major trends affecting physician practices over the past decade. It will then discuss future directions in three specific clinical domains: primary care, oncology and surgery. Finally, it will discuss directions for policy to help ensure that medical practice remains attractive and that physicians are invested in creating a high performing US health care system.
