Home-Based Healthcare in the 21st Century
As health systems enter payment models where they are accountable for managing total spending within a predetermined budget target, they will begin devoting resources to reducing potentially avoidable hospitalizations and nursing home stays. As the baby boom generation retires and our primary care physician workforce is stretched thin, there is a growing need for clinical models that allow older Americans with complex chronic conditions and physical impairments to age in place and receive medical care, when appropriate, in their homes. This forum will examine innovative programs developed by integrated health systems, health plans and entrepreneurs to provide primary care, emergency care, and even hospital-level care in patients’ homes. It will discuss the potential for new technologies to facilitate sophisticated care in patient’s home at reduced cost. The forum will conclude with a discussion of financial and regulatory barriers to effective home-based care and policy options that could increase its availability and effectiveness.
